Mentor Survey
PEERS is conducting a follow-up survey of students who served as mentors for the Peers Educating Peers about Positive Values© (PEP) program from 2002 to the present. When you served as a mentor, you may have completed a paper survey. You also may have participated in our on-line follow-up survey last year. We would like to invite you to complete an on-line follow-up survey this year. This survey will help us to further evaluate the PEP Program and its long-term effects on our mentors.
You should have received a letter and/or email that gave you your login ID and password.
We value your response, so once you have completed the survey, your name will automatically be entered into a drawing where the grand prize is an I-Pod.
